The Pathfinders Coalition

The purpose of the Pathfinders Coalition is to bring together parents, teachers and adult service staff who are engaged in person-centered transition planning on behalf of young people with disabilities throughout the New York City area. Everyone invited to be a member of the coalition is directly involved with at least one person in transition. The coalition meets eight times a year to share ideas, developments, resources and expertise related to inclusion-oriented transition work

At meetings we: get the word out about "good" stories, share information about individualized living arrangements and options, discuss ways to get more families involved and active in person-centered transition plans, and learn more about self-determination.
