Community Supports

These services were developed as an alternative to group-oriented day programs and are for people who need long-term supports to enable them to become involved in civic, volunteer and recreational activities in their communities.

Meet David
David, an outgoing Brooklynite in his twenties, loves photography and music. His weekly schedule keeps him busy. On Mondays, he volunteers at the Supreme Court in lower Manhattan. On Wednesdays, accompanied by a Job Path support worker, he volunteers at the Brooklyn College radio station from 9-1 where he is the official Music Librarian. Fridays, he is at the Life Path Center at Job Path where he meets with other participants and support workers. And on Tuesdays and Saturdays, you can find him at his paid job at the Foodtown in Bayridge, a placement made through Job Path's Supported Employment program.
Community Supports
Rather than remaining on the sidelines, Job Path's Community Support Team helps people actively participate in the life of their communities. As part of their daily schedules, Osvaldo has a weekly pick-up basketball game under the Brooklyn Bridge, and Rita regularly takes aerobics classes at the Von King Recreation Center and volunteers at the Wayside Senior Center in Brooklyn. Keith, who loves animals and music, volunteers at BARC (Brooklyn Animal Resource Coalition), where he walks dogs. On his way home, he and his support worker often stop by local music stores, where Keith is known by fellow bass guitar aficionados.

Community Supports connects people to volunteer jobs and social and recreational activities in their communities, supports them in these activities and helps them build relationships with the people they meet. Job Path looks for service and religious organizations, social centers, parks, and gyms where people can pursue their specific interests. The goal is to facilitate each person's unique plan. (Some people have also elected to find paid jobs through Job Path's employment programs.) Initially, Job Path is the person's main resource, along with a few close relatives and friends. But as individuals become known in their communities, they develop friendships with the people they encounter from classes, gyms, volunteer work and other activities, and their support circles expands.

Eligibility
New York City residents 18 or older who have a documented developmental disability, such as mental retardation, epilepsy, autism, cerebral palsy, or a traumatic brain injury that was incurred before age 22, can receive community services from Job Path. Must be eligible for the Medicaid Home & Community-Based Waiver.
