Medicaid Service Coordination (MSC) assists people with developmental disabilities to find the services they need to lead healthy and fulfilling lives.
The focus in MSC is on assisting each person to identify his or her goals and desires and to achieve them - from having a home, a job, pursuing interests and hobbies, a "circle of friends" and participating in community life. In addition, the service coordinator ensures the person has access to medical, clinical and other services that promote good health and safety.
The service coordinator is a strong advocate who helps each person put together a life plan and makes sure that all the paid and non-paid supports are working together on that person's behalf. The coordinator, who is involved with the person's family as well, helps locate all the supports and services that person needs. For example, the coordinator may help a person get connected to a doctor, solve a housing issue, straighten out problems with SSI or other entitlements, and find an employment program.
New York City residents 18 or older who have a documented developmental disability, such as mental retardation, epilepsy, autism, cerebral palsy, or a traumatic brain injury that was incurred before age 22, can receive community services from Job Path.
